BY THE STATE OF FLORIDA:

A
- Z Child Development Center, Inc. is
licensed and regulated by the State of
Florida. The license is displayed in
the Reception Area.
CHILD
DEVELOPMENT CENTER HOURS:

The
center is open from 6:00 a.m. to 6:30
p.m., Monday through Friday.
REGISTRATION:

There
is a one time $40.00 registration fee
upon enrollment of your first child
and a $35.00 fee for each and
subsequent child that is due at the
time of enrollment. This is not a
refundable fee.
ART
SUPPLY FEE:

There
is also a $25.00 art supply fee per
child that is due on September 1st of
every year, this fee is used to
replenish art supplies for the entire
center for the upcoming school year.
TUITION
AND FEES:

The
week's tuition is due on the first day
of attendance and Monday of each week.
In the case of tuition not paid on
time, a late fee of $20.00 per day
will be added as from Wednesday of
that week.

Tuition
is guaranteed until May 1 of
the upcoming year. We sit
down and assess tuition fees
in March and if we determine
an increase is needed then a
notice will be giving out in
April. We are committed to
providing quality child care
at an affordable cost.

A
fee of $25.00 will be charged for a
returned check and A - Z Child
Development Center, Inc. will have
the option to refuse any further
payment by check. If a child is
enrolled as before school, after
school or before and after school
ONLY, and the School classes are
canceled due to emergency break such
as fire, storm or the school is
scheduled for half-day, or teacher's
planning-day, there will be no
additional charge over the weekly fee.
In case the period extended longer
than twenty four hours, a prorated
daily fee will be charges for any
additional day based on weekly fee
charged by the Center for Kids
enrolled for Vacation Period.
REFERRAL
POLICY:

Our
Referral Plan is a complete A to Z
opportunity for our Parents to
draw-down on the cost of their weekly
tuition. Other Centers provide less
services and no incentives, we give
more and serve well. Though we watch
our bottom line, it is not at the
expense of our Parents'.

Our
A to Z Referral Plan can not be
matched by any Center in town, and
that is a fact..! Our plan enables
parents to save $5.00, per child
referred and registered, off of their
weekly tuition. This
credit continues for as
long as the referred child

is
enrolled at A-Z. With the A to Z
Referral Plan our Parents can earn
free tuition for their own child or
children by bringing in new
enrollments that stay with the Center.
This Policy ONLY applies to referrals
after May 15, 1998.

information.
ABSENTEE
POLICY:

A
- Z Child Development Center
had
established policy on children
absentees from the Center. If a child
is absent due to Parent being on
vacation, illness or any other events,
a full week's tuition is charged if
the child attends any portion of the
week. In order to reserve the child's
enrollment spot, for a vacation,
illness or other events of one week or
more, the Parents are asked to pay
half of the weekly tuition for each
week of absence. Enrollment will
automatically be withdrawn if the fee
is not paid.
OBSERVATION
SYSTEM:

All
parents will like to see how their
child interacts and plays with other
children when they are away, A - Z
Child Development Center has the
answer. Each classroom is equipped
with a camera which allows you to see
your child on the monitor, the
observation screen is located in the
Reception Area of the Center.
NUTRITIOUS
MEALS:

Our
children are served a breakfast or
morning snack for those having
breakfast at home, lunch, and an
afternoon snack are for no additional
fee. The weekly menu will be displayed
for your review. We request that you
provide the food for your infant and
toddler who is not eating table food
yet.
We
advise that you do not let your child
bring food or candy into the Center.
If the child is having a birthday, you
are welcome to provide a
"treat" for each child in
the child's classroom. Please make
arrangements with the Director on the
date and time.
INFANTS:

Parents
will need to provide formula, baby
food, diapers, and wipes for each
infant they enrolled. Have the
infant's name written on the items or
container thereof.
Please
have at least two complete changes of
clothing for your infant each day of
the week.
MEDICATION
POLICY

Upon
a signed permission by the Child's
Parent or Guardian, A - Z Child
Development Center, Inc. will
administer medication. The medication
must be dated, labeled, and must be
prescribed for the child. Medication
will be administered as instructed by
the prescribing Physician ONLY.
Parents must complete an
"Authorization for
Medication" each day the child is
to receive medication.
MEDICAL
FORMS:

All
Parents are required to have their
child's current medical and
immunization records prior to the
child's enrollment in the Center. The
Parents can obtain these required
records from their physician: HRS Form
3040 (yellow) and Form 680 (blue).
CHILD
DISCIPLINARY PROCEDURES:

A
- Z Child Development Center, recognizes
that discipline should be a teaching
process rather than a punishing
process. A - Z does not allow
the use of any physical punishment,
and children will not be subject to
discipline which is severe,
humiliating, or frightening.
